- Funding rate: 100% (equity-free funding) - Budget conditions / limitations: Each project can receive a maximum of EUR 5,000, paid to the Living Labs. - Payments: The grant is disbursed at the end of the collaboration, upon reception of the final report.

Funded by the European Union, EVOLVE2CARE focuses on HealthTech experimentation practice to standardize and enhance the collaboration between Living Labs and innovators in the context of the Transitional Care sector. The project will provide innovators with access to services for testing and validating research and products. EVOLVE2CARE will realize its ambition from different interlinked perspectives (technological, social, economic, financial) reflected in 4 Key Objectives - Foster user-centric research and support technology upscaling - Enable deployment and commercialization - Demonstrate pan-European testing by fostering coordination and regulatory action - Encourage knowledge sharing and Collaborations EVOLVE2CARE focuses on three Use Cases - Homecare Monitoring Solutions - Hospital Discharge Management - Ageing Population & Care Transitions

Eligibility
- Innovators can be: SMEs/companies, independent researchers or group of researchers, research organisations, Associations, Foundations or Clusters in the health care sector. - Living Labs must be ENoLL-certified and located in EU member states or associated countries. - Technology Readiness Level (TRL) between 3 and 6. The consortium partners count an already established community of over 480 Living Lab members in Europe and worldwide engaging with more than 500 innovators and researchers, 50 investors, and a massive reach of diverse stakeholders such as policy makers regulators and end-users, that can be a lever not only for the EVOLVE2CARE project but for the whole of EIC KIC communities.

Additional information
The Open Call (OC) will follow the process flow as defined below: 1. Registration on the Accelup Platform: All applicants must first register on the Accelup platform. 2. Matchmaking Process (Accelup Platform): The matchmaking process will also take place on the Accelup platform, where mini-consortia will be formed. Miniconsortia can be composed of: ➢ 1 Innovator and 1 Living Lab ➢ 1 Innovator and 2 or more Living Labs, depends on the innovator types of services 3. Registration on Sploro’s Platform: Once the mini-consortia are formed on Accelup platfrom, registration on Sploro’s platform is required for mini-consortia teams. EVOLVE2CARE Consortium Partners recommend to the mini-consortia teams that the application on the Sploro’ Platform to be open by the innovators rather than the Living Labs.
